Welcome to Forest Medical in Mansfield incorporating Rosemary Street Health Centre and Oak Tree Lane Surgery
Our Services
We know our phone lines are often very busy. When possible, why don't you book an appointment online? Just visit our appointments area, it might save you some waiting time.
We also have an active Patient Group. They help us to offer patients the right services. If you are interested in joining, just let us know.

Meet our Team
We have 2 GP Partners, 5 Salaried GPs and a team of Nurse Practitioners, Clincial Pharmacists, Practice Nurses and Healthcare Assistants
The Reception Team look after patients, their appointments, prescriptions and all the stuff that flies around, whilst our Admin Team look after the massive amount of administration involved in running a busy, practice.

Practice Information
Parking
Parking is available around the back of the building but it is very limited. It is advisable therefore to park in the Civic Centre car park next door where you get 30 minutes free. There are 4 disabled bays situated next to the Pharmacy
Disabled Access
Both buildings are DDA compliant. Oak Tree Lane Surgery is one storey and there is a lift available at Rosemary Street Health Centre for anyone with mobility issues. Loop systems are situated on the front reception desks to assist those with hearing impairment.
Facilities
Rosemary Street Health Centre is joined to the pharmacy to provide efficient provision of medication.
Dementia Friendly Practice
We strive to be a Dementia Friendly Practice and you will see appropriate signage around the building, a Dementia Friendly area, and Twiddle Muffs are available on request.

The GP Patient Survey is an independent survey run by Ipsos MORI on behalf of NHS England. The survey is sent out to over two million people across England. The results show how people feel about their GP practice.
